Health and Well-being Coach

The Health and Wellbeing Service aims to help patients in the Nottingham West Primary Care Network take more of an active role in their own health care through structured Health Coaching consultations.

There is a focus on changing detrimental lifestyle behaviours with the intent of reducing health risks, improving self-management and symptoms of chronic conditions.

The Health and Well-being Coach can guide and support patients to set realistic and achievable health related goals to improve quality of life and overall health and wellbeing.

The criteria for the service includes all patients living with Diabetes or pre-diabetes and all patients living with a respiratory condition.

The Health Coach offers 1:1 support as well as group exercise sessions and support sessions. Sessions can be conducted virtually, face to face or over the phone according to the patients preference.
