Health and Wellbeing Coach.

The Health and Wellbeing Service helps patients take an active role in their health care through structured sessions.

The aim is to change lifestyle behaviours to reduce health risks and improve self-management and symptoms of chronic health conditions.

The Health and Wellbeing Coach guides and supports patients to set realistic, achievable health related goals to improve quality of life and overall health and wellbeing.

Patients eligible for this service are people with diabetes or pre-diabetes and/or people with a respiratory condition.

The Health Coach offers one to one support as well as group exercise sessions and support sessions. Sessions can be online, face to face or over the phone according to the patients preference.

Support with the NHS App and Online Services.

Support is available to help patients manage their healthcare by using the NHS App and Online Services to find health adviceorder prescriptions, check test results, manage appointments, and much more.

Patients can take control of their healthcare, access services at a time that suits them, and reduce the need for unnecessary trips to the doctor’s surgery.

Paul and Pauline are our local Digital Inclusion Team and they understand this technology can be daunting. They provide one to one support in a friendly, relaxed way. They can help you set up the NHS app on your device*, verify your identity, and provide help with issues such as resetting your password. They can also advise how to use the key features of the NHS App.

A weekly Drop-in Session is held at the Church Walk site every Wednesday from 9am until 12 noon, no appointment necessary.

Features of the NHS App:

  • Access to health advice, symptom checker and 111 online
  • Receive messages from your GP
  • Appointment management: view, cancel, and book GP appointments (*where available), view hospital appointments (*where available, dependant on hospital dept.)
  • View and manage repeat prescriptions, change your nominated pharmacy, view opening times of your nominated pharmacy
  • Access your COVID Vaccination records.
  • Obtain test results, access to your GP health record, submit sicknotes (*where available), and track your health
  • Access Linked profiles to manage the healthcare of any dependants from your own account (i.e. if you are acarer, or the parent of a child under 13)

*Registration for the NHS app requires an email address, internet access, a smart phone or tablet, and a valid photo ID (such as a driver’s licence, passport, or citizens card). Users must be 13+.

Liberty Leisure Exercise Referral Scheme

This health scheme provides opportunities for people living with underlying mental or physical health conditions to lead a more active lifestyle and provide safe and effective access to various activities.
